We may also need to contact your health fund to inform us of your rebate eligibilities.

Your information will only be disclosed to a third party for eye test reminder letters. In these cases, a non-disclosure contract has been signed on their behalf. Your information will be destroyed as soon as the letters are printed. For the short time your information is stored, it is encrypted by a 256 bit encryption code (the same type used by the military) in a secure building with on-site security guards and limited access.
Correct Information
We require up to date information in order to provide you the best possible service and care. If you find that the information we currently hold about you is incorrect, please contact your local store or email headoffice@1001optical.com.au with your local store and correct details.
Storage of Information
Your personal information is only accessed by authorised persons on a secure system to only be used for approved purposes. We exercise the best possible caution to ensure that your information is not misused.
